Sliding Fee Discount
We offer medical, dental, and behavioral health services on a sliding scale. Patients may be asked to pay a minimum amount at the time of their appointment. Any remaining charges will be billed to the patient. Our staff will work with you to determine your eligibility for our sliding fee discount. Access does not deny requested health care services and shall not discriminate in the provision of services because of inability to pay for services.
Medicaid and BadgerCare
Both are accepted for medical, behavioral health and dental services at all of our clinics.
Medicare
Medicare is accepted at our medical clinics.
Private Health Insurance
All of our clinics accept a variety of private insurance plans, including Dean Health Plan, Group Health Cooperative and Quartz, in addition to some dental insurance plans. Some plans may require prior approval.
Co-payments
Patients who have coverage that requires a co-payment will be asked to pay that amount at the time of service.
Good Faith Estimate
Under the law, health care providers need to give patients who don’t have insurance or who are not using insurance an estimate of the bill for medical items and services. For more information about the Good Faith Estimate, contact Patient Services at (608) 443-5480.
