Access’ Sliding Fee Discount program helps make medical and dental care affordable.
Can I apply for the Sliding Fee Discount?
Any patient of Access Community Health Centers can apply for the Sliding Fee Discount, regardless of their ability to pay.
How is eligibility for the Sliding Fee Discount determined?
The level of discount that a person or family is eligible for is based on the combined household income and family size. A person or family’s income is compared to the current Federal Poverty Levels to determine if a patient pays a nominal fee for services or if they pay a percentage of the charges of services. Those with incomes between 100 percent and 200 percent of the Federal Poverty Level will receive a certain percentage discount on services. Those with incomes at or below 100 percent of the Federal Poverty Level will pay a nominal fee.

In addition, Access will help patients who qualify for other insurance or assistance programs apply for them.
How do I apply for the Sliding Fee Discount?
You can apply for the Sliding Fee Discount as part of your appointment. Ask to speak to a Patient Services Specialist, who will walk you through the application process.
What documents do I need to apply for the Sliding Fee Discount?
You will need to provide verification of income and family size to receive the Sliding Fee Discount. Documentation can include:
Income verification:
- Your most recent pay stub.
- Your most recent year’s tax filing.
- A letter from your employer stating weekly, monthly, or annual earnings.
- An earnings Verification form filled out by your employer.
- An unemployment earnings statement.
- Your Supplemental Security Income (SSI) or Social Security Disability Insurance (SSDI) information.
- Your annuity check stubs.
- Your pension check stubs.
- A statement of no income.
Verification of family size:
- A birth certificate showing that an applicant is the parent of a child.
- Hospital footprints indicating the applicant is parent of a child.
- School enrollment forms showing the applicant is the parent/guardian of a child.
- Court documents showing guardianship of a child.
- Your most recent year tax fling indicating dependents, spouse, or domestic partner.
- A marriage license showing your spouse.
- Your bank statements with names of both adults indicting spouse or domestic partner.
- Your lease/mortgage with both adult names listed indicating spouse or domestic partner.
If you have no income, you are still eligible to apply for the Sliding Fee Discount.
Who is considered family under the Sliding Fee Discount?
Family is an individual, domestic partner or spouse, dependent children, and unborn babies. Parents and legal guardians of adult children over age 18 can chose to include their adult children on the discount program if they can prove dependency through their most recent tax return.
What services are covered under the Sliding Fee Discount?
Access offers discounts applied to the fees for medical, dental and behavioral health services, as well as at clinic pharmacies at Access through UW Health Pharmacy Services.
How much will I pay for services covered under the Sliding Fee Discount?
The amount you will pay will depend on your discount level. Our team of Patient Services Specialists will work with you to determine your level.
For how long am I covered by the Sliding Fee Discount?
The Sliding Fee Discount will be valid for one year from the date of the application for patients with income, and for 6 months from the date of the application for those with no income or unemployment income.
